Survey Consent Information

Thank you for agreeing to participate in our survey. This is a consent form for participation in a Diversity Research & Consulting Toronto pilot research project that is supported by the Ministry of Labour, Immigration, Training and Skills Development. 

Your participation in this research study is voluntary. The survey will be anonymous and voluntary. We do not ask or identify any individuals who plan to participate in this survey. If you have any questions about the survey, incentives or privacy policy, please contact the researcher

INFORMED CONSENT:

You must be aged 18 years or older to participate in this survey and reside in the Province of Ontario. By entering the survey, I indicate that I have read the information provided and agree to participate.

PURPOSE OF THE RESEARCH: 

The purpose of this pilot study is to understand educators’ perceptions and experiences with generative artificial intelligence (AI) platforms and tools. You will be asked to fill out an online questionnaire. The questionnaire may take up to 5 minutes to complete. No identifying information will be collected in the questionnaire.

Participating in this research may not benefit you directly, but will help us better understand major themes educators are experiencing with AI in Ontario. Any findings or analysis conducted based on this research will be published on this website and free to download. There is a minimal risk that respondents participating in the survey will feel shame, worry or embarrassment based on use or non-use of AI.

VOLUNTARY PARTICIPATION:

I have been informed that my participation in this pilot study is voluntary and that I am free to withdraw or discontinue participation at any time prior to submitting my responses. I have been informed that data collected for this study will be retained by the investigator for the length of the research and analyzed even if I choose to withdraw from the research. I have been informed that withdrawal after submission of the survey is not possible given the anonymous nature of the survey. DRAW INCENTIVE RULES AND REGULATION:

After submitting your survey responses, you will be directed to a separate survey to enter your contact information to be entered into a draw for a chance to win a $500 giftcard. Your odds of winning are based on the number of individuals who participate in the survey. Your email will be stored separately from your survey responses and permanently erased once the draw is complete. The survey will close on December 31, 2023.  

The winner of the $500 giftcard will be selected in a random draw from eligible entries on December 31, 2023. The draw will be held 10 business days after the closing date and be conducted by an impartial third party. The researcher will contact the winner by email. If the winner has not responded in 7 days, the winner will forfeit the prize, and another draw will be held to determine a winner. The prize must be accepted as awarded or forfeited and cannot be redeemed for cash. Please note that in order to win the $500 giftcard, the selected entrant must correctly answer a time-limited skill-testing mathematical question.

PRIVACY POLICY 

Diversity Research & Consulting Toronto is committed to respecting the privacy of survey respondents to our surveys. All personal information created, held, or collected by is protected by Canada’s Freedom of Information and Protection of Privacy Act. We are committed to protecting your personal information and respecting your privacy. Personal information is defined as any details that will enable you to be identified, such as ID numbers, telephone numbers, address, email address etc. We do not sell, rent or exchange any personal information supplied by you to any third party. Nor do we use any of the information you provide for direct marketing or other non-research activities. All personal identifying information (i.e., email address) are used only for the administration of prizes and is stored separately from survey responses. All non-identifying survey data is used solely for evaluation and / or research purposes. All survey data are transferred and carefully stored on encrypted, password protected, secured network drive (electronic) or in a locked cabinet (paper resources). It is our policy to monitor our internal procedures regularly to ensure compliance with the relevant statutory requirements in all that we do, including the Freedom of Information and Protection of Privacy Act.

CONFIDENTIALITY: 

Any information learned and collected from this study in which I might be identified will remain confidential and will be disclosed ONLY if I give permission. The investigators will work to keep my personal information confidential. To help protect my confidentiality, the survey is anonymous and will not contain information that may personally identify me.
